How to make a referral to a Clinical Psychologist at Fremantle Clinical Psychology

If you want to make a referral to a specific Clinical Psychologist or to the practice in general, there are several ways you can do this. You can:
- write a referral letter and send it to the address on the “Contact Us” tab and ask the patient to phone us for an appointment
- write a referral letter and ask the patient to bring this with them when they make an appointment or
- you can print out and fill in the simple referral sheet on the next page and FAX it to our practice, and we will either contact the patient directly for an appointment or they can ring us.
- a valid referral needs to include:
- the patient’s details
- your name
- provider number
- practice address
- the date of the referral
Without these details on FCP invoices, patients will be unable to claim from Medicare. As well, if you are making a referral under the Better Outcomes initiative, or other rebatable schemes, your letter needs to state this to meet Medicare requirements.
